First, start by making a ball of glass at the end of your rod.

Squash the ball into a flat disk using flat masher pliers.

I'm going to pick up some frit to make the mushroom more interesting.

Melt the frit into the glass.  

Squash it again into the glass.

Now, heat the tip of a second rod, while keeping the flat disk hot.  

Melt the second rod tip into the center of the disk.

Melt the rod that has the disk and separate the disk from its rod. There are two says of doing this. You can clip the rod at the disk, or melt it and pull it apart, as in the below picture.  

Once you've removed the disk from the first rod, you can smooth out where the first rod was attached. 

Soften the disk at the end of the second rod you attached it to, and gently plunge it into the marver hole. For this demonstration, we are using the largest mushroom shaper hole

Smooth your new mushroom as preferred. 

Clip the mushroom stem at your desired length and allow it to fall onto a ceramic blanket. 

 Heat the bottom of the rod where you just cut it off so you can shape it flat.

Flatten the bottom of the mushroom stem so that it will stand up. I'm using my flat masher pliers to flatten it, but any flat surface, such as a flat marver, will work. Put your mushroom in your annealer or blanket. Below, I'll show you some other mushroom shapes that these mushroom marvers can make.
